How to Complete the New Forms

IRB Application 2009As you may know, there are new forms available for the IRB application process. These forms are based off of feedback from the research community here at Capella. The new forms will assist you through the IRB review process by clarifying commonly overlooked or mis-answered questions and what may be needed for IRB review.

Older versions of the IRB application and supplemental forms will be accepted until April 30. Starting May 1st, all submissions submitted must use the new version of the IRB application and supplemental forms. They are located in IRBNet and in the Research and Scholarship Center on iGuide. If you are unsure which version you are using, check the date of publication located at the end of the first page. The new version is dated 3/31/2009.

Completing the New Forms

To complete the new forms, you will need to simply click once on the grey shaded field and start typing. To check a box, click once on the grey-shaded box and an “x” to appear. Many researchers copy and paste information from the dissertation proposal into the IRB application. The best way to copy and paste information is to use the short-cut keys. First, copy (ctrl-c) the information from your proposal and then click once on the grey shaded field where you want the information to go. Next, paste (ctrl-v) the information.

Most IRB Applications submitted are incomplete. Here are some tips on how to avoid simple mistakes.

Print the forms. Sometime reviewing a paper copy will reveal much more than the electronic version. If you print the new forms, the grey-shading will disappear.
Peer review. Always have your mentor review your forms. Yet, another set of eyes is always nice! Have your peers review the IRB application to see if anything is missing.
Use the check list. At the end of the IRB application there is a check list. Check off each section as you complete it.
